Risk Management is built directly into
Wellsoft v11 and
is offered throughout the entire product. It is not a separate,
billable module. Risk Management is, and has always been, inherent
to the design of the software.
Below are some examples of the many ways in which Wellsoft provides risk management.
- Drug/Drug & Drug/Allergy Checking - Medications ordered, prescribed, or dispensed in the ED can now be checked automatically or on demand against medications taken and known drug allergies for possible drug-drug interactions or allergy/hypersensitivity reactions. An integrated link in the Wellsoft system to Gold Standard, a leading developer of drug information databases, provides immediate, up-to-date, clinically-relevant information on all U.S. prescription drugs, as well as off-label uses and dosage, herbal supplements, nutritional and over-the-counter products and new and investigational drugs.
-
Medication Reconciliation - Decrease the possibility of wrong drug and wrong, duplicate or missed dosage errors using the Wellsoft v11 medication reconciliation feature. Designed to enhance patient safety, an expanded and enriched medications table includes a source column to capture the reliability of medication information. An MD Review column has been added to document home medications instructions. The patient’s home medications instructions are then printed along with prescriptions if discharged and with orders if admitted.
- Medical Record Flagging - Wellsoft v11 queries its own table structure to find previous records. The patient's record is flagged on the Tracking Display to indicate the patient has had previous visits. This helps identify multiple visits within a hospital-defined time interval.
- Patient Care Plans - A longitudinal record of a custom patient care plan can be stored within Wellsoft v11. Patients with care plans (i.e. sickle cell patients) are automatically identified on the Tracking Display when they arrive in the Emergency Department. Clinicians and specialists can IMMEDIATELY access the Care Plan right from the Patient Tracking Display.
- Robust Discharge Instructions - Wellsoft Discharge Instructions are the gold standard in Patient after-care instruction. With a few keystrokes, clinicians can provide detailed instructions for patients to take with them when they leave the ED.
- Centralized Location for Medications & Allergies - Wellsoft v11 prominently displays patient medications and allergies at the top of every patient record. Allergies are displayed in red for immediate recognition. The location of this information is in the same place on every chart.
- Real-time Alerts - Alerts on the Tracking Display indicate to clinicians that something needs their attention. Alerts can indicate re-evaluation is needed, insufficient documentation in a patient record, or a reminder to check vital signs. Hospital-defined time intervals can be color coded on the Tracking Display to alert clinicians as to the last time patient vitals were checked.
- Mandatory Data Fields - Any data field in Wellsoft can be made to be mandatory. This can enforce compliance for pain scale, domestic violence screening, vital sign collection, JCAHO required fields or any state mandated, hospital policy and practice standard.
Electronic Documentation:
Wellsoft v11 Clinical Documentation Templates have built-in required data entry elements related to specific levels of care/service. 90% of all required clinical documentation is contained within the templates and pop-up boxes/menus. Standard of Care is incorporated into the templates. The Wellsoft system is designed to be robust and fast, without interrupting the clinicians with reminders that slow down the documentation process.
Order Entry/Tracking and Results Reporting:
The Wellsoft v11 Order Entry module incorporates a sophisticated prompting system to avoid risks, while efficiently processing Orders. Critical questions are asked as a result of placing certain orders ~ i.e. when placing an order for an X-RAY, the clinician is asked if the patient is pregnant. If a clinician orders 3 X-RAYS on the same patient, Wellsoft v11 will store the answer to the pregnancy question so the clinician is not asked 3 times for the same patient, thus preserving speed while managing risk.
As a converse example, the "reason for study" prompt must be answered for certain orders and when 3 different tests are ordered, the reason for study question will be asked 3 separate times. In an effort to preserve the speed of placing orders, the previous answer will appear as the default ~ if the reason is the same for all 3 tests; time is not wasted re-typing the same answer.
Status of Orders is constantly visible on the Tracking Display ~ the elapsed time from when the Orders were placed, the time it takes to process the orders, and then the time waiting for results to be reviewed all appear on the tracking screen. Clinicians never have to wonder about the status of an order and the results can be reviewed with one keystroke. Abnormal results are flagged on the Tracking Display with an alternate color to alert clinicians.
